DynaForms is a web application that enables you to create and test simple questionnaires, invite people by email to fill them in and review the results.
Each questionnaire (or form) is divided up into pages and questions. Each page can have any number of questions and each question can be one of the following types :-
- Intro (No input) This is used as an introduction (or comment) and does not accept any input.
- Text Box This allows for simple inputs like names, telephone numbers or email addresses.
- Text Area This allows much larger inputs like addresses or comments.
- Drop-down box This limits the input to one item from a list.
- Yes / No This limits the input to either a yes or no.
- Score 1 ~ 10 This limits the input to a score of one to ten.
- Date This allows for date input. Years, months and days are in the form of drop-down boxes.
Each questionnaire can be styled in your own corporate look and feel.
